Note: Schools using the pdf form must have a current adobe acrobat program installed on their computer, otherwise it will not allow you to submit the completed form. To use the pdf version, simply complete the pdf form, and submit to complete your registration. If after filling out the pdf form, it will not allowed you to submit, save on your drive and email the registration form as an attachment to franklin@math.colostate.edu
For those schools without adobe acrobat on their computer, use the word doc form. Simply complete the form, save on your drive and email the registration form as an attachment to franklin@math.colostate.edu If you prefer to send the registration form along with your registration check, mail to the following: Christie Franklin If you have questions, please call Christie at 970-491-6452.
